For some time now our legacy billing system has been billing all customers under the name of OCOSA Communications, LLC this has been a mistake and we have tried to correct to no avail with the software producer company of our billing software. Our legal entity name registered with the State of Oklahoma is OCOSA Communication, LLC. Our doing as business name (d/b/a) registered with the State of Oklahoma is OCOSA Communications as this was the name we started with in 2003.
We would appreciate it if you could update your records to reflect this matter. We also have taken steps to procure this situation by transitioning to a new billing system completely. For tax reasons we will be pleased to submit an IRS W-9 form to update your records by request only.
